Prior to your first visit at NYU Langone Fertility Center, please reach out to your insurance company to discuss coverage for fertility services. When you speak to the customer service representative at your insurance carrier, please ask about any copayments, deductibles, lifetime maximums, prior authorizations, precertifications, fertility case management requirements, or excluded services or procedures that may apply.

If you have coverage for your initial consultation, payment will be due onsite for any copay, coinsurance, or deductible related to your visit. Once you have determined your care plan alongside your physician, payment will be due for any additional copay, coinsurance, or deductible related to your fertility treatment cycle. All copay, coinsurance, or deductible payments are determined by your insurance carrier.

Most insurance carriers will require verification of coverage before you are authorized to start treatment. The customer service representative for your insurance company may direct you to an online benefits verification or authorization portal. Please complete your insurance verification and pre-authorization as soon as possible to avoid future delays in your treatment. Additionally, please be aware: prior authorization does not guarantee future payment. An insurance carrier may deny benefits depending upon the status of your benefit when the claim is processed. Patients are responsible for any copay, coinsurance, deductible, excluded service, or excluded procedure fees not covered at the time of service or upon notification.
